Application Package Instructions
22 Teams of two competitors each will be accepted for this competition. Team slots will be filled as COMPLETE Application packages are received. A complete Application Package shall consist of
- payment of $150 per competitor
- completed Application for angler and co-angler
- Release of Liability Waiver for angler and co-angler, and
- Talent Release form for angler and co-angler

Completed Application Packages may either

or downloaded using links below, completed including an e-signature (type in your signature and requested information) and e-Mailed as attachments to S_greenbacks at chapter509tu.org (at = @). |
Payment may either be made by check or via PayPal, below. Application Package will not be considered to be complete until all forms and payment are on file.

If you do not have a team partner, submit your completed Application Package, and a team partner will be provided. Team placement will not be guaranteed until payment, application, waiver, and talent release forms are recieved. Frostbite Fish-Off Awards Party & DinnerIf you wish to bring guests for the Awards Party (dinner included, cash bar), please include payment with your Application Package at the rate of $25 per person, or pay at the door. Anglers, sponsors and event volunteers may attend for free. Friends, spouses, and other interested persons may attend by payment at the door. Note that payment is required whether guest has dinner or not due to catering agreement. |
